I declare that I am the beneficiary and concurrently also the beneficial owner of the proceeds that are the subject of the application, and that the proceeds have not – by means of a substitute payment in lieu of dividend or any other quid pro quo, in whole or in part – benefitted, nor will they benefit another person who: 1) has a lesser right (or no right) to an offset, refund or reduction than myself and 2) their original share position is retained directly or indirectly after the dividend date.
